At MAR Wellness, your privacy is a top priority. We are committed to protecting your health information in accordance with federal and Florida state laws, including the Health Insurance Portability and Accountability Act (HIPAA).

Protected Health Information (PHI) includes any information about your health, health care services, or payment for health care that can identify you. This includes:

· Billing: To process payments or claims (note: MAR Wellness is a fee-for-service provider)

· By Your Request: If you authorize us in writing to release your information to a specific person or organization
❌ What We Will Not Do
· We will not sell your PHI.
· We will not use your PHI for marketing purposes without your explicit written consent.
· We do not share your PHI with third parties unless required by law or authorized by you.

As a patient, you have the right to:

· Limit or restrict how your PHI is used or shared

· File a complaint if you believe your privacy rights have been violated

⚠️ Important Reminder
MAR Wellness is a telehealth provider. If your provider determines that virtual care is no longer appropriate, you may be referred to an in-person primary care provider or local emergency department for follow-up care. Responsibility for your comprehensive medical care ultimately remains with your local physician or primary care provider.
