Telehealth Informed Consent Agreement

At MAR Wellness, we are dedicated to providing accessible, high-quality care through telehealth. Telehealth involves delivering health services through secure electronic communication—such as video or phone consultations—when you and your provider are not in the same location.

Services will be provided by a board-certified Autonomous Advanced Practice Registered Nurse authorized to practice independently under Florida Statute §464.0123.

Your Ongoing Care

While you may receive care through telehealth with MAR Wellness, the responsibility for your comprehensive care remains with your local clinician or primary care provider (PCP). If your provider determines that telehealth is no longer sufficient for your care, you may be referred back to your PCP or instructed to seek care in person, including emergency services when appropriate.

Benefits of Telehealth

Using telehealth services with MAR Wellness offers several potential benefits, including:

·        Faster access to care and reduced travel or waiting time

·        Flexible care from the comfort of your home or other safe location

·        Streamlined medical evaluation, communication, and follow-up

Potential Risks

Although telehealth offers convenience, there are some risks to consider:

·        Technical difficulties (e.g., dropped calls, poor video quality) may delay or interfere with your care

·        Rarely, missing or incomplete medical information could result in diagnostic or treatment errors

·        Loss of data or privacy breaches may occur, though MAR Wellness uses HIPAA-compliant platforms

The most significant difference between telehealth and in-person care is the absence of physical exams. You always have the right to opt for traditional, in-person care instead.

Patient Responsibilities and Agreement

By receiving services from MAR Wellness via telehealth, you understand and agree to the following:

1.       You are responsible for providing a complete and up-to-date medical history. You agree to update your health record regularly and at least annually.

2.       MAR Wellness does not prescribe controlled substances or medications not appropriate for telehealth.

3.       MAR Wellness services are not intended for emergency or life-threatening conditions. If you believe you are experiencing a medical emergency, you must call 911 or go to the nearest emergency room.

4.       Services may be denied at the provider’s discretion. MAR Wellness reserves the right to refuse care if it is determined to be inappropriate, unsafe, or a misuse of services.

5.       You are not guaranteed a prescription. Prescriptions are based solely on clinical judgment.

6.       All visit fees must be paid in full at the time of scheduling. MAR Wellness operates on a fee-for-service model and does not accept insurance for provider services.

7.       Medications prescribed by MAR Wellness can be filled using your insurance at your chosen pharmacy, but the clinical services themselves are self-pay.
